I hereby grant Park National Bank (“Park National”) permission to use my testimony, made on behalf of Park National, in connection with publicizing and promoting Park National. I grant Park National, its representatives and employees, the right to use my name, brief biographical information, and the testimonial in various marketing initiatives in any electronic or printed media or communications, as edited or combined with other media, and for any advertising, promotion, or publication, including Park National’s website, social media, and other electronic or physical media or collateral. I give consent to the use of this Released Testimonial while knowing and understanding that my name, comments, and other identifying factors may be revealed to the general public. However, Park National may not make known to any party in any medium my known or previously known location, email, or physical address, or any other contact details, such as my phone number. I grant Park National ownership of the testimonial and brief biological information. I waive any right to inspect or approve the finished printed or electronic media that may be used in conjunction with the testimonial, now or in the future, whether that use is known to me or unknown. I waive any right to royalties or other compensation arising from or related to the use of the testimonial. I release and forever discharge Park National, its employees, agents, assigns, and any designee from any and all claims and demands arising out of or in connection with the use of such testimonial, including, but not limited to, any claims for defamation or invasion of privacy. This consent and agreement shall be binding on my heirs, assigns, agents, and my estate.

I am of legal age (18 years or older), warrant and represent that the information on this Testimonial Release is true and accurate, have read and fully reviewed and understand the contents of this Testimonial Release, and agree to the Testimonial Release terms, including the Participant Authorization and Release provision above.
